What to Check Before Attending In-Person Service & Small Group Gatherings

We are asking families and individuals to use these symptom and close contact/exposure questions.

If you, or anyone coming to service (or a small group gathering) with you, answers "yes" to any of these questions, we ask that you would join us online for service this week (or join your small group virtually)!

SYMPTOMS

If you have any of the following symptoms, that indicates a possible illness and puts you at risk for spreading illness to others. Please check yourself (and anyone from your household that will be attending service, or a small group gathering, with you).

___Temperature 100.4 degrees Fahrenheit or higher when taken by mouth
___Sore throat
___New uncontrolled cough that causes difficulty breathing (for people with chronic allergic/asthmatic cough, a change in your cough from baseline)
___Diarrhea, vomiting, or abdominal pain
___New onset of severe headache, especially with a fever
___New loss of taste or smell
___Muscle or body aches
___Chills

CLOSE CONTACT/POTENTIAL EXPOSURE

___Had close contact (within 6 feet of an infected person for at least 15 minutes) with a person with confirmed COVID-19.
___Traveled to or live in an area reporting large numbers of COVID-19 cases.
___Live in an area of high community transmission.
